Maltrattamento e abuso nell’infanzia quali fattori di rischio di suicidio in età adolescenziale e adulta

Eventi avversi e in particolare una storia di abuso (specialmente abusi sessuali) si associano a un aumento del rischio per un’ampia varietà di problemi di salute e disturbi del comportamento e costituiscono importanti fattori di rischio per il suicidio aumentando la probabilità di atteggiamenti autolesivi, ideazione suicidaria e tentativi di suicidio [5,16-18,20,30,46].

In letteratura è ampiamente riportata una elevata prevalenza di disturbi psichiatrici tra soggetti che hanno vissuto esperienze di abuso fisico, sessuale e/o psicologico/emozionale nell’infanzia [6,39]. In particolare, gli abusi subiti nell’infanzia sono stati associati allo sviluppo del Disturbo da Stress Postraumatico (PTSD, post-traumatic stress disorder), al disturbo borderline di personalità, a sintomi dissociativi e, disturbo depressivo [10,22,37]. Tutte le forme di abuso (fisico, psicologico e sessuale) sono state trovate associate ad un’elevata prevalenza di disturbi depressivi [14,19,25,26] che, come è noto, costituiscono un fattore di rischio per il suicidio.

Va sottolineato che gli effetti sulla salute psicologica si possono manifestare anche molti anni dopo gli eventi, in età adulta o anche anziana [4,9,33].

Più studi hanno documentato un’associazione tra abusi e violenze subite in età infantile e ideazione suicidaria e tentativi di suicidio sia nel periodo dell’infanzia/adolescenza [24,34,42] che nel corso della vita [45] e un rischio maggiore di tentativi di suicidio è stato trovato nel caso di abusi fisici e sessuali [2,7,12,20,23,32,40,44]. È stata anche documentato un forte effetto dose-risposta tra esperienze di abuso e la probabilità di sviluppare sia un disturbo depressivo che comportamenti suicidari nel corso della vita: il rischio aumenta al crescere della frequenza degli abusi e del livello di violenza implicata [1,9,11,15,32,38,41]. L’abuso sessuale nell’infanzia sembra essere un fattore di rischio per suicidio persino più forte di quello rappresentato da una storia di comportamenti suicidari nella famiglia [21].

Un studio svedese pubblicato a novembre 2019 su Eur Child Adolesc Psychiatry [36] riporta che le vittime di abusi sessuali hanno un maggior numero di co-morbidità e un maggior ricorso ai servizi sanitari rispetto ai coetanei sia negli anni precedenti che nell’anno successivo alla diagnosi di abuso sessuale. Una delle più comuni cause di ricorso ai servizi sanitari nel primo anno dopo la diagnosi di abuso sessuale è il tentativo di suicidio [36].

Indagini condotte in Italia su campioni (non probabilistici) di popolazione studentesca riportano una prevalenza di abusi sessuali subiti nell’infanzia che va dal 12,5-34,1% e percentuali ancora più elevate di abusi emozionali e fisici [8,35].

Non sono a nostra conoscenza, ad oggi, studi di popolazione rappresentativi a livello nazionale volti a valutare l’associazione tra abusi/violenze nell’infanzia e suicidi/tentativi di suicidio.

Il fenomeno della violenza sui minori nel nostro Paese resta un fortemente sottostimato [28] e l’implementazione di un sistema di monitoraggio su questo tema sarebbe altamente auspicabile. Una formazione specifica delle figure “sentinella” (operatori scolastici, pediatri, medici di medicina generale, assistenti sanitari e psicologi, ecc) sarebbe altresì di primaria importanza per l’individuazione dei casi di violenza familiare ed extra-familiare, finalizzata alla prevenzione e alla presa in carico e dei bambini e ragazzi vittime di violenza e di coloro a rischio di suicidio.
